If you apply for health insurance or other products through our Website, we may ask you to provide us with personal information and/or health information relating to you and any family member who will be included on your application. This information will be used by the insurance company, licensed agent, association, insurance agency and/or broker, benefit provider, third-party administrator, government insurance entity, insurance exchange, and/or other persons or entities involved in the sales, marketing, and/or administration of insurance, association, or other insurance related products to process your application.
